The Center for Healthy Aging is a multi-disciplinary treatment center, focused on meeting the healthcare needs of seniors. The clinical staff includes board certified geriatricians, psychiatrists, internists, clinical nurse specialists and social workers. The team provides treatment to the frail elder, chronically ill, and cognitively impaired in collaboration with primary care physicians. Services are covered by Medicare and most major insurance carriers.
GERIATRIC CARE MANAGEMENT

NortheastLink provides complete case management and care planning for seniors and serves to link patients and families with the services and resources they need as they age. Care planning aids families in making important decisions in an effort to avoid having to make difficult decisions about the care of a loved one during a time of crisis. Specialized assessments for individuals and families coping with Alzheimer’s disease are also available.

Spectrum Adult Day Health Program is recognized by the Alzheimer’s Association of Massachusetts and provides specialized daytime care for adults with Alzheimer’s disease or related memory disorders. Physical and cognitive assessments are regularly scheduled for clients and socialization is promoted through a variety of failure-free activities. “Spectrum at Home,” in-home services, are offered through a partnership with North Shore PRN.
IN-HOME SERVICES

North Shore PRN is a home healthcare agency providing assistance and support to seniors to help them live safely in their homes. Assistance can be provided with nursing care, personal care, meal preparation, light housekeeping, medication management, and many other activities. Services are provided by Certified Home Health Aides, Certified Nursing Assistants, Licensed Practical Nurses or Registered Nurses, depending upon the needs of the client. North Shore PRN also offers a specialized home care program, “Spectrum at Home,” for clients with dementia. Caregivers have all been fully trained and received Alzheimer’s Association certification.

Northeast HomeCare is a Medicare certified home health agency dedicated to providing outstanding quality physician-ordered home health services to patients on the North Shore. Qualified, professional staff provide skilled nursing services, rehabilitative therapy services, including physical, occupational and speech therapy, medical social work and home health aide services. Services are typically covered by Medicare and other health insurance programs.
PERSONAL EMERGENCY RESPONSE SERVICE

Lifeline is a Personal Emergency Response Service that provides access to emergency services 24 hours a day at the push of a button. This service offers peace of mind, security and independence for individuals who live alone, are recuperating from an illness or injury or who are physically challenged. Beverly Hospital’s Lifeline has been serving residents of the North Shore for over 20 years.

Assisted living at The Herrick House is a lifestyle option offering independence and companionship in a supportive, safe, secure environment. Gracious planning and design make the 88-unit Herrick House residential community a distinctive place to live. The Herrick House staff includes 24-hour nursing, resident care aides, a social worker and a recreation director, all of whom work together to meet the needs of each individual resident.

Located on the campus of Addison Gilbert Hospital in Gloucester, Seacoast Nursing and Rehabilitation Center offers a friendly and elegant setting and an extensive program of short-term rehabilitation and skilled nursing care which returns patients home at their highest level of functioning. Seacoast offers specialized Alzheimer’s care in a secure, therapeutic environment where residents are cared for by compassionate, dedicated staff with special training and certification in Alzheimer’s patient care.
