Details Schedule Full-time Shift Days Hours 40 hours per week Description Work as an integral part of the Referral Center team in coordination and support of the patient referral process for Northeast HomeCare, Beverly Hospital’s certified home care agency. Collaborate with the hospital liaisons, outside vendors and multiple departments to meet the client clinical needs and ensure timely admissions to certified home care services. Track patient referrals in a database using technologies such as Excel, Microsoft Word, E-Discharge, Allscripts and Meditech. Education RN licensed to practice in MA
BSN, MSN preferred
Skills Management experience. Strong organizational, communication and customer service skills. Understanding of Medicare, Medicaid and Managed Care environment. Strong disease/diagnosis assessment skills. Problem solving, customer service and computer skills. Attention to detail, accuracy and ability to prioritize and re-prioritize tasks.

Details Schedule Weekdays or weekends available Shift Hours Flexible Description Responsible for agency admissions, evaluations and follow-up visits scheduled during the weekend to patients in their residence. Initiate a Plan of Care and appropriate rehabilitative procedures and coordination of services. Communicate with Case Managers and covering physicians for patient concerns. Education - Graduate of an accredited program in Physical Therapy
- Current professional license to practice Physical Therapy in Massachusetts

Details Schedule Per Diem, Flexible weekdays or weekends Shift Days or early evening Hours Flexible Description Home care visiting nurse needed to do patient admissions, work with the Case Managers to coordinate patient skilled nursing, preventative and rehab nursing care. Counsel patients and families to educate and participate in treatment plans. Communicate with physicians, Case Managers and other personnel regarding patient care.
